エビデンスに基づいた安全性の高い術式の提供を目指して
⽇本全国に102院（2026年2⽉現在）を展開する美容クリニック TCB東京中央美容外科（理事長：寺西 宏王、以下「TCB」）の中上勝一朗医師は、経結膜脱脂術における圧迫法・牽引法の術後合併症について比較検討し、論文「Evaluation of a compression method for transconjunctival fat removal: safety and surgical utility compared to a conventional traction approach」（経結膜脱脂術における圧迫法の評価:従来の牽引法と比較した安全性と手術的有用性）を発表しました。

＜論文要旨＞
背景
経結膜脱脂術は下眼瞼の膨隆に対して広く行われている美容外科手技であり、結膜を牽引して視野を展開する牽引法が一般的に用いられている。しかし、この方法には下眼瞼皮膚の穿通や下斜筋損傷などの合併症のリスクがある。我々は、これらの合併症を回避するために、眼窩を圧迫することで鈍的剥離を可能にし、エネルギーデバイスを使用せずに術野を展開する圧迫法を開発した。本研究では、圧迫法と牽引法における安全性を検証するために術後合併症について比較検討を行った。
方法
2024年3月から2025年2月までに単一の術者によって当施設で施行された経結膜脱脂術60症例を対象とし、術野展開に使用された手技に基づいて圧迫群(n=30)と牽引群(n=30)に分類し、術後1ヶ月間の合併症について後方視的解析を実施した。
結果
両群間でベースラインの患者背景に有意差は認めなかった。術後合併症の発生率は圧迫群で10.0%(3/30例)、牽引群で6.7%(2/30例)であり、両群間に有意差は認めなかった(P=0.640)。全ての合併症は軽度であり、再手術を要さず自然軽快した。眼窩出血、感染、視力障害などの重篤な有害事象は両群において認めなかった。
結論
圧迫法は牽引法と同等の安全性を示し、特に経験の浅い術者にとって実行可能な代替法となり得ることが示唆された。経結膜脱脂術における視野展開方法として、圧迫法は簡便で潜在的により安全なアプローチを提供し、臨床実践および外科研修における有用性が示された。
