＜論文要旨＞
背景
埋没式重瞼術（Buried Suture Double Eyelid Blepharoplasty：BSB）は、東アジアにおいて最も頻繁に行われる美容外科手術の一つである。我々は以前、BSBに上眼瞼脂肪除去術（Upper Eyelid Lipectomy：UEL）を同時施行することで、術後早期合併症が減少することを報告した。しかし、その際の平均観察期間は7.5か月にとどまっており、重瞼線の持続性に対する中長期的な効果は明らかではなかった。本研究では観察期間を延長し生存時間解析の手法を用いて、UEL併用が埋没糸に関連するイベントの発生を減少させるかどうかを評価した。
方法
本研究は単一施設・単一術者による後ろ向きコホート研究であり、2024年1月から12月にかけてBSBを受けた連続102名の患者を対象とし、2026年2月まで追跡した。対象患者は、BSB単独群（モノセラピー群、n=61）とBSB＋UEL併用群（コンビネーション群、n=41）に分けた。主要評価項目は、埋没糸に関連する事象（重瞼線の消失または弛緩に対する再手術または抜糸）の発生とした。重瞼線が保たれている期間はKaplan-Meier法を用いて推定し、log-rank検定により比較した。
結果
追跡期間の中央値は19か月（四分位範囲16–22か月）であった。埋没糸に関連する事象は17名（16.7%）に発生し、モノセラピー群では14名（23.0%）、コンビネーション群では3名（7.3%）であった（P=0.056）。重瞼線が保たれている期間は、コンビネーション群で有意に長かった（log-rank検定、P=0.037）。コンビネーション群で発生した3例はいずれも整容的な修正希望によるものであり、埋没糸に起因する合併症は認めなかった。
結論
UELの同時施行は、中期的な観察期間において重瞼線が保たれている期間の有意な延長と関連しており、重瞼線の持続性の向上が示唆された。
